CHAN is the only HCCN in New Hampshire, with members in Texas and Vermont, as well. The mission of CHAN is to enable our member agencies to develop the programs and resources necessary to assure access to efficient, effective health care for all clients in our communities, particularly the uninsured, Medicaid, and medically underserved populations and other vulnerable populations. We provide Health Information Technology for our FQHC members including the hosting and application support for an EMR, patient portal, patient engagement tools, interfaces and revenue cycle management. The CHAN Data Warehouse provides reporting and data visualization, registries and dashboards that support clinical quality measurement, population health reporting and analysis, operational and financial reporting. We also provide quality improvement training and technical assistance.
