Orange County Partners in Health – OCPH

In August of 2016, the Coalition of Orange County Community Clinics (The Coalition), was awarded a federal grant and the designation of Health Center Controlled Network (HCCN) for 10 of its 26 members by the Health Resource and Service Agency (HRSA), and has since grown to a cohort size of 12 Federally Qualified Health Centers and lookalikes.
The HCCN’s name is the Orange County Partners in Health (OCPH) and it’s comprised of the following organizations in Orange County California: Camino, Central City, Families Together of Orange County, Hurtt Family Clinic, Korean Community Services, Nhan Hoa Comprehensive Care Clinic, Family Health Matters, Serve the People, Share Our Selves, St. Jude Neighborhood Health Centers, Southland Clinic Integrated, inc., and UCI.
So far our activities have focused on implementing Health Care Information Exchange (HIE), Data Aggregation, and Data Analytics (reporting) to focus on Clinical Quality Improvement (CQI) for the cohort. We have an aggressive work plan to: connect all the HCCN members to a regional HIE to support point of care information exchange, aggregating data for longitudinal data analysis across the cohort, implementing an analytics and reporting tool for use by HCCN staff and individual members of the cohort at the network level, and CQI collaboration on education, skill building, and improving measures for a specific set of HP2020 goals.
