The current Hawaii Health Center Controlled Network (HiHCCN) is a project under the Hawaii Primary Care Association (HPCA), funded by grant number H2QCS30267. HPCA has been fortunate to have been awarded this grant for four funding cycles. There are ten health centers in Hawaii participating with the HiHCCN. The purpose of the HiHCCN is to support participating health centers to collaborate on the use of health information technology (health IT) to improve operational and clinical practices. Specifically, the HiHCCN exists to support health centers in leveraging health IT to increase their readiness and participation in value-based care through:

  • Enhancing the patient and provider experiences;
  • Advancing interoperability; and
  • Using data to enhance value.
