Health Center Connections

Description

The Health Center Connections HCCN is organized and supported by the Community Care Network of Kansas, the federally designated Primary Care Association for Kansas.

Connections provides member health centers with a population health and analytics system, licenses to health IT tools, technical assistance, educational resources, training, and the opportunity to learn from and support each other through the successes and failures of individual experiences.

Community Care is a statewide association made up of a network of community care clinics striving to make sure all Kansans have access to high-quality, community-based, whole-person health care close to home.
